Spots on a pillowcase or sheet are unsettling, especially when nobody in the room can explain them. The good news is that the marks themselves tell you a lot. Their colour, their shape, where they are and what happens when they get wet all help separate bedbugs from the many other things that mark bedding.

Small Red Marks and Smears

Bright or rusty red marks are fresh blood. With bedbugs they come from two things. A bite can ooze a little after the bug has left, leaving a tiny spot where the skin was touching the sheet. And a bug that has just fed and is squashed by someone turning over leaves a short reddish smear.

Neither is proof. A single red spot is more often from a scratched bite of another kind, a spot or a patch of dry skin, a small cut, a nosebleed, or the edge of a period. What makes red marks suspicious is repetition: new ones most mornings, in different places, with itchy bites on skin that was uncovered.

Dark Dots That Look Like Ink

These are the marks that matter more. Bedbugs leave droppings made from digested blood, and they look like dots from a fine black or dark brown felt-tip pen. On fabric they soak in slightly rather than sitting on top, and if you rub one with a damp cloth it smears rather than flaking off.

Dark spotting on the sheet is worth following up, but the place to confirm it is not the sheet at all. Pull the bedding back and look along the mattress seams, the piping, the corners and the joints of the frame. Clusters of these dots there, often with pale shed skins or tiny white eggs, are one of the clearer signs.

Other Causes Worth Ruling Out

  • Scratching eczema, spots or other bites at night can leave small red marks with no insect involved.
  • Mould and damp can leave dark specks, usually in patches rather than along seams.
  • Make-up, hair dye, cocoa or tea can leave brown marks that wash out easily.

When the Marks Point to Bedbugs

Put the marks together with anything else you have seen. Taken together, these make bedbugs likely: dark spotting in the mattress seams or frame joints, red marks reappearing on fresh bedding, bites in lines or small groups, and anyone who has spotted a small, flat, brown insect. If you catch one, keep it in a jar or on sticky tape.

Do bedbugs always leave blood on the sheets?

No. Some people have them for a while with no marks on the bedding at all, which is why the mattress seams are the better place to look.

Will the dark spots wash out?

They usually fade in a hot wash, but they can stain. Washing the sheet does nothing about bugs in the mattress and frame.
